These sample forms are designed to provide further guidance and to illustrate the various ways in which the online LED form can be completed. Given the infinite number of possibilities, these examples are certainly not exhaustive. However, we hope that they will address some of the more common problems and questions that might arise as contributors fill in the LED form.
